At the Hospital OSI Araba ESI in Vitoria, the first clinical case of the REBOA technique to control postpartum hemorrhage was successfully performed using the Cobra-OS device, distributed by InnoVasc Integral Solutions, S.L.
Case Context:
The patient presented with placenta accreta, a high-risk condition for intrapartum hemorrhage. Thanks to the collaboration with the gynecology department, our section implemented this innovative technique.
Three-Phase Procedure:
1️⃣ Epidural approach by the anesthesia team in the recovery unit (URPA).
2️⃣ Right femoral access in the interventional radiology suite using an 8F introducer, allowing placement of a balloon catheter and a Cobra catheter in parallel, ready for embolization if required.
3️⃣ Transfer to the gynecology operating room for the cesarean, where Dr. Javier Maynar performed controlled inflations of the balloon for aortic occlusion:
- 6 inflations of 7 minutes each, separated by 2 minutes (except between the 2nd and 3rd, which were 20 minutes apart due to no observed bleeding).
Results:
- Newborn: 3 kg male infant.
- Outcome: Mother and child discharged 72 hours later, without complications.
Innovation and Multidisciplinary Collaboration:
This case showcases the potential of the Cobra-OS for managing both postpartum and traumatic hemorrhages, emphasizing the value of teamwork among gynecology, interventional radiology, and anesthesia services.
